Summary
We are seeking an Occupational Therapist to join our team at Certus Healthcare Management. In this role, you will play a vital part in developing and implementing individualized treatment plans for our residents, focusing on enhancing their quality of life through rehabilitation and therapeutic interventions.
This position comes with a starting pay rate of $XX an hour
The typical schedule for this opportunity is Monday through Friday with flexible start and end times based on caseload. This opportunity is a PRN, as needed, position but may require you to see clients at multiple communities in a dedicated area.

Responsibilities

Conduct assessments to develop personalized occupational therapy plans for residents.
Implement therapeutic interventions aimed at improving daily living skills and overall functionality.
Collaborate with interdisciplinary teams to ensure comprehensive care for each resident.
Monitor progress and adjust treatment plans as necessary based on resident needs.
Educate residents and their families about therapy goals and techniques for continued improvement.
Participate in discharge planning to facilitate smooth transitions for residents returning home or to other care settings.
