Position Summary

Sullivan Park Care Center is seeking dedicated and detail-oriented Licensed Practical Nurses (LPNs) to join our Medicare team.
Key Responsibilities

• Complete and follow up on MD orders in collaboration with the interdisciplinary team.
• Document daily skilled nursing notes in accordance with Medicare guidelines.
• Perform and record weights and nutrition at risk.
• Conduct comprehensive resident evaluations, ensuring timely assessments and updates.
• Lead and participate in infection prevention protocols, including monitoring, reporting, and staff education.
• Maintain compliance with all federal, state, and facility policies, procedures, and documentation standards.

Qualifications

• Current LPN license in the state of Washington.
• Prior experience in skilled nursing, long-term care, or Medicare documentation strongly preferred.
• Strong clinical assessment, organizational, and time-management skills.
• Knowledge of MDS processes and Medicare regulations is a plus.
• Ability to work independently and as part of a team.
• Excellent communication and documentation skills.

Pay: $38.50-$43.50 per hour depending on experience and education.
