Occupational Therapist (Healthcare)

Pine Creek Care Center is seeking a dedicated and compassionate Licensed Occupational Therapist to join our interdisciplinary care team. In this flexible role, you'll help patients regain independence, improve daily functioning, and enhance their quality of life through customized, evidence-based occupational therapy interventions. Key Responsibilities

Patient Evaluation & Care Planning

  • Conduct comprehensive occupational therapy evaluations and functional assessments.

  • Develop and implement individualized treatment plans in alignment with physician orders and patient goals.

  • Collaborate closely with physicians, nursing, and therapy teams to ensure consistent, holistic care.

Direct Treatment & Supervision

  • Provide skilled therapeutic interventions focused on ADLs, functional mobility, cognitive rehabilitation, and adaptive techniques.

  • Supervise Occupational Therapy Assistants, therapy aides, and students per applicable state regulations.

  • Support and train Restorative Aides in therapeutic activities in collaboration with nursing staff.

Team Collaboration

  • Participate in care planning meetings, utilization reviews, and interdisciplinary team conferences.

  • Clearly communicate patient progress, needs, and discharge plans with the care team and families.

Documentation & Compliance

  • Complete evaluations, daily notes, progress reports, and discharge summaries using NetHealth or similar EMR platforms.

  • Ensure documentation complies with state, federal, and insurance guidelines.

Family Training & Discharge Support

  • Educate patients and caregivers on home safety, adaptive equipment, and home exercise/skills maintenance programs.

  • Recommend and help coordinate provision of assistive devices and home modifications (e.g., grab bars, adaptive utensils).

Equipment Oversight

  • Monitor and report therapy equipment needs to maintain a safe and effective treatment environment.

Qualifications

  • Education : Bachelor's degree in Occupational Therapy required; Master's or Doctorate preferred.

  • Licensure : Current California Occupational Therapist license in good standing.

  • Certifications : Current BLS certification required .

  • Skills : Strong clinical reasoning, documentation accuracy, and communication skills.

  • Experience : Familiarity with NetHealth or similar EMR platforms is a plus.

  • Mindset : Patient-centered, collaborative, and passionate about quality care.

Physical Demands

  • Frequent walking, standing, bending, and lifting.

  • Ability to occasionally lift or support over 100 pounds with assistance .

  • Must have sufficient vision, hearing, and mobility to perform therapeutic duties.
