Social Services Director - Alhambra Post Acute
Location: Martinez, CA
Pay: $20 to $28 per hour, DOE
Schedule: Full-Time

Alhambra Post Acute is looking for a Social Services Director to join our leadership team. This role is key to ensuring our residents and families receive the support, resources, and guidance they need throughout their care journey.

Responsibilities

• Oversee the social services department and supervise social services staff
• Support residents and families with psychosocial needs, care planning, and discharge planning
• Develop and maintain relationships with community resources and agencies
• Participate in resident care conferences and interdisciplinary team meetings
• Ensure compliance with state and federal regulations related to social services
• Provide education and support to residents, families, and staff regarding care transitions and available resources

Qualifications

• Degree in social work, psychology, or related human services field preferred
• One year of supervised social work experience in a healthcare setting required
• Knowledge of discharge planning processes and community resources
• Strong communication, organizational, and leadership skills
• Compassionate and resident-focused approach
